how to add new mobs or item to server

If you're running an Episode 2 server, simply add a DB Entry for the new item and generate a new OMI. It won't have anything super fancy, but it gets the job done.

If you're running an Episode 3+ server, you're going to have to do the same thing; however, there isn't an OMI generator openly released to the general public such as with Episode 2. Can't help you there.(I never brought up an Ep3 server, I'm uninterested personally)
